Course Reserves - Course Creation Form

Course Creation Form

Use this form to initially request the creation of the Course Record to which your Course Reserve Items will be listed under. You may proceed to the Course Reserves - Reserve Item Addition Form upon the completion of this form. This form only needs to be fill out once per semester.

1. Instructor's Name - First and last name of the primary course instructor. *
2. C.W.I.D. Number - The Campus Wide Identification number of the primary course instructor. *
3. Extension - Phone extension for the primary course instructor.
4. Department - Department course is under. *
5. Course Title - Title of the course for which the items will be put on Reserve. *
6. Course Number - The course number of the course. *
7. Class Number - Please enter the 4 digit number of the class. *
8. Course Password (For E-Reserves Only) - Password to access your online reserves course.
9. Semester - Semester the course is being taught. *
10. Date Needed - Date which items need to be on Reserve by. *

Use this form to add items to your course. You may do this at any point in the semester. Please allow two business days for processing. Please note that not all fields may be applicable to your request and that any relevant missing information may delay the completion of the request. 3. Author/Editor - Please enter the name of the Author or Editor of the item. *
4. Call Number - Please enter the Call Number of the item.
5. Date Published - Please enter the publication date of the item.
6. ISBN or ISSN - Please enter the ISBN or ISSN of the book or journal.
7. Chapter and/or Pages - FOR ELECTRONIC RESERVES: Please enter the Chapter and/or pages to be place on Electronic Reserve.
8. Volume/Issue - Please enter the Volume and/or Issue number of the Reserve Item.
9. Class Number - Please enter the 4 digit class number associated with the course for which the item will be put on Reserve. *
10. Instructor C.W.I.D. - Please enter the Campus Wide Identification Number of the primary instructor. *
